• Initial Consultation: $300  (cash/pre-pay rate). *The full billed rate if not paid before appointment is typically $470.  This depends on the complexity and length of visit.
• Follow up appointment: $150 (cash/pre-pay rate). *The full billed rate if not paid before the appointment is typically $310. This depends on the complexity and length of visit.
• ADHD testing: $200
• Forms/paperwork taking more than 5 minutes to complete: $200
• Injectable medication administration: $40
• Memo: $200

Claims submitted to insurance are per the insurance contracted rate (which may be more or less than the cash pre-pay rate).  Patient will elect either self pay or insurance prior to visit, but not both.  

Inquiry of insurance benefits is an estimate.   Insurance explanation of benefits is definitive.

• Initial visit must be in person, but follow up appointments can be done virtually through secure video.
• We require in person visits at least every 3 months in order to obtain vital signs such as blood pressure, pulse, oxygen saturation, weight, etc.

• Appointments can be scheduled by calling the clinic (281-724-7980) during regular business hours.
