For Initial Consultation, we ask that you print and fill out all forms and return prior to your consultation. We also request that you please bring to your appointment any medication that you are currently taking.
If you are unable to print out forms, we offer New Patient packets for pick up at our office. If you are unable to complete forms prior to your appointment, we ask that you come in at least 30 minutes prior to your scheduled appointment to fill out all forms.
